How to Refer a Patient
A referring physician, patient or family member may contact a transplant/leukemia coordinator, or transplant physician to discuss a potential referral, to arrange a consultation or hospital transfer.
Patients referred to NSH BMT and Leukemia Program will be provided with an in depth consultation and comprehensive review of treatments options. Treatment plans are designed to meet the unique needs of each patient. We will coordinate your pre- transplant as well as your post - transplant care with your referring physician. If pre or post-transplant disease management therapy is recommended, you can be assured treatment therapy will be the most current and state-of-the-art.
The transplant and leukemia coordinators will facilitate the consultation process with you and your referring physician's office. You will be sent educational information in advance of your scheduled appointment. New Patient Consult Brochure, Autologous Patient Information Brochure, Allogeneic Patient Information Brochure.
Acute leukemia patients can be set up for a direct admission to the inpatient unit or a hospital transfer seven days a week.
